Bankers Life long-term care claims

What does Bankers Life need with a long-term care bill?

Bankers Life wants an itemized bill showing dates of service, cost per service, and units per service. For a nursing home or assisted living, it also needs a new Proof of Residence form (form 186484) every month, completed on or after the last day of that month. It rejects photocopies of a prior month's form. Don't send a month's charges until that month is over.
